This activity will involve planning, implementing recording and reporting an activity undertaken with a group of clients or client in the care setting.

- Cookery session
- Or the activity can also be a social activity i.e. outing to the shops, garden centre, to a social function, to the zoo etc.
- Or any other activity carried out in the care setting
Your assignment will be assessed on the following:
- Through organisation and preparation of the task, including identification of patients’ needs
- Careful execution of the task
- Effective communication throughout the task
- Effective use of relevant safety and health practices
- A comprehensive record of the task
Construct your report under the following headings:
- Introduce the client you are going to be working with providing relevant background information.
- Preparation for task {to include client preparation, environment, materials, equipment, staff, transport, consultation with manager and consent obtained from client and manager}
- How did you determine the level of assistance that the client would require? {Reference to a care plan, Activities of living}
- List the equipment needed to complete this activity efficiently
- What communication skills did you use when communicating with client during the planning and execution of task? {Verbal and non-verbal communication to be addressed}
- Outline in detail how this task was carried out from beginning to end, to include the rationale for interventions, demonstrating good client care and observation skills
- Outline the health and safety and Infection Control issues involved in this task and the rationale for the same
- Outline how you communicated with other team members. Was this task recorded in clients care plan and if so what information was recorded and why?
- Evaluate the task taking into consideration what was good about the experience and what did not go so well both from your perspective and also the patient’s/client’s perspective
