I am required to write an assignment title ‘A PERSON CENTRED APPROACH DURING AND OF LIFE CARE’ Where I will describe how to provide a person-centered approach to caring for an 84-year-old woman who lives in a nursing home where I am employed as a care assistant, her name is Mary and she is in the last days of her life.

I will address each symptom and needs to include the provision of supportive and holistic care for her, I will state the importance of offering choices and prioritizing needs when caring for Mary, I will outline the importance of effective and sensitive communication when caring for Mary, I will Describe the importance of working well as part of a team when caring for the patient and I will finally outline the need for care and support for those affected by Mary’s end of life care and her death including her family, the care staff and fellow residents in the nursing home.
