Complete a community profile and health needs assessment of your placement area (i.e. Preceptor PHN’s geographical area) using the guidelines and pro forma template (Buckley & Mulcahy 2011) which are available on Blackboard. 15th September 2017 Using one of the ‘Priorities for action in the coming year’ identified from the profile of the area (on the last page of the pro forma), identify and specify potential public health nursing interventions from the literature that can be used to address this individual/family/population health/wellness/illness problem. Provide a rationale for the use of the named intervention(s) in the area and discuss/specify the outcome measures that can be used to measure the effectiveness of the intervention (s). Your discussion should remain cognisant of the diversity and resources in profiled area and remain feasible in the profiled area, i.e. both parts of the assignment need to be closely integrated.
