Reducing any damage is central to risk assessment and succeeding risk management plans (Royal college of nursing, 2013). It was observed that since the patient had a history of self-injury and attempted suicide, the nurse had to carry out a risk assessment. Some of the most common themes of risk management include self-harm, self-neglect, aggression, potential and violence . This was the same themes that were discovered in Mark’s case. The primary reason for carrying out a risk assessment is to establish and reduce future problems and come up with interventions and management strategies. When carrying out the risk assessment, there are several indicators that we considered by the nurses such as circumstances, intention, history and means, alongside protective factors that Mark had and any risk that he could pose to himself or others. Risk assessment should be delicately developed while minimising harm to self and to others, detailed, comprehensive and shared with members of the MDT team
