Following the admission procedure, all new patients are required to undergo a physical health examination (Royal collage of psychiatry, 2013). The National Early Warning Score (NEWS) tool was used to address the gaps in Mark’s physical health. According to the Royal College of Physicians, (2012) this tool identifies shortfalls in pules, oxygen saturation, blood pressure, blood sugar, levels of consciousness, temperature and respiration. Any parameters outside their normal range identified by NEWS are those considered to be risky on people with serious mental illness diagnosis (NICE, 2014a CG7.2.8.5). Although the NEWS tool was a good tool to assess Mark’s physical health, consideration should have been given to the Health Improvement Profile (HIP) as a better tool to use. The reason is that it integrates the information on the NEWS tool such as sleep patterns, teeth, smoking status, urine, weight, height etc. (white et al., 2009; White, 2015). The aim of using the HIP tools as a better option is too speedy the evidence-based action for any parameters that are identified to be at risk and direct the nurse to work with the patient on what action to take next (White, 2015). The Malnutrition Universal Screening Tool (MUST) was used as part of the physical health assessment to assess Mark’s situation. It assessed nutrient intake and determined if he was malnourished or at risk of being obese and to develop a managed care plan as per the recommendations of the Malnutrition Advisory Group
